To complete the CDM training, you must attend both sessions.


2016 - 2017 Tuition is detailed below. Tuition rates are subject to change.

  • For fellows who have completed their bachelor's degree: $10,500
  • For two or more fellows from the same health care system or fellows from an active Institute member organization: $9,500
  • Tuition covers the cost of course materials and program site expenses.

Future tuition rates are subject to change

Discounted tuition rates are available for fellows referred by members of the American Leadership Council for Diversity in Healthcare.


Payment Terms

Student must pay the entirety of their tuition prior to starting the current year cohort. Full payment is required to receive their CDM credentials and certificate.

Admissions Requirements

  • Participants must have obtained a Bachelor's degree
  • Current employment in the role of a diversity practitioner or health care professional with a minimum of three years experience in the field of diversity or a related field. This may include experience as a manager, human resource professional or practicing clinician.
  • Completion of the online application for admission. You will be required to upload the following documents to our database.

Required Documents

Please complete the enrollment form and submit a pdf or doc version of the required documents to Jetaun Mallett at to begin the registration process.


Resume: Employment information, including name and address of employer, dates of employment and nature of work or title.

Letter of Support or Sponsorship: Must be from a direct supervisor confirming and describing the title and role as a diversity leader within the participant's organization.


Personal Statement:  In 1 ½ to 2 pages, describe your purpose for completing the Certificate in Diversity Management in Health Care program. Please discuss your Leadership/management experience focused on efforts to improve diversity and reduce health care disparities.
