Discounted tuition rates are available for fellows referred by members of the American Leadership Council for Diversity in Healthcare.

 

Payment Terms

Student must pay the entirety of their tuition prior to starting the current year cohort. Full payment is required to receive their CDM credentials and certificate.

Admissions Requirements


Required Documents

Please complete the enrollment form and submit a pdf or doc version of the required documents to Jetaun Mallett at jmallett@aha.org to begin the registration process.

 

Resume: Employment information, including name and address of employer, dates of employment and nature of work or title.


Letter of Support or Sponsorship: Must be from a direct supervisor confirming and describing the title and role as a diversity leader within the participant's organization.

 

Personal Statement:  In 1 ½ to 2 pages, describe your purpose for completing the Certificate in Diversity Management in Health Care program. Please discuss your Leadership/management experience focused on efforts to improve diversity and reduce health care disparities.
